Our Medicare and Medicaid plans and services enhance health and independence by providing a Nurse Practitioner or Care Manager (depending on the individual's needs) as your personal guide through the complex world of health care. All of Evercare's Medicare Advantage Plans come with Part D prescription drug benefits. These plans help protect clients from the rising costs of medications they may need to stay healthy.

To be eligible for these plans, prospective clients need to be eligible for Medicare.

Health Plans for People with Limited Incomes
If you need to stretch your health care dollar, Evercare provides a number of state-specific health plans for people enrolled in Medicare and who are eligible for Medicaid benefits (or "dual eligible"). These plans provide access to coordinated, personalized care to help you use your benefits to stay healthy.

Health Plans for People in Nursing Homes
Evercare helps you access personalized, individualized, coordinated care for nursing home residents. Evercare supplements nursing facility services with an added layer of care, more covered benefits and a Nurse Practitioner for no extra monthly health plan premium.

Health Plans for People with Specific Long Term Illnesses
If you have one or more illnesses including heart disease, respiratory disease such as asthma, high blood pressure, diabetes or dementia, Evercare can help you manage not only your illness, but also your overall health and well-being.

Arcadian Health Plan offers Medicare Advantage health plans to Medicare-eligible individuals. Enrollees in plans can choose from options with no or low premiums, depending on their health needs.

When Medicare laws were modernized by Congress in 2003, the private sector was given a greater role in administering Medicare services. In addition to regular Medicare, beneficiaries now have the choice to enroll in Medicare Advantage plans. These follow a managed-care model that uses a network of local doctors and hospitals for enrollees to choose from.

This practice can reduce health care costs while ensuring excellent care and access to specialists. Because of the more efficient Medicare Advantage model, beneficiaries are able to enjoy a greater range of benefits compared with Medicare, reducing the need to carry an additional supplemental insurance policy to cover Medicare gaps.

Care Improvement Plus is a unique Medicare Advantage Part D (MA-PD) health plan offering specialized services specifically built for Medicare beneficiaries living with diabetes, heart failure, and/or end stage renal disease—people whose wellbeing can be very dependent on the good health decisions they make.

Care Improvement Plus provides more benefits and services than Original Medicare or many traditional Medicare Advantage plans and the chance for better health and better quality of life. Our services make positive health management a daily priority.

For people living with diabetes, heart failure and/or end stage renal disease, good health relies on more than just taking medication and going to the doctor occasionally. These conditions require a complete health plan that takes a person’s specific conditions and lifestyle issues into consideration. Care Improvement Plus facilitates care with existing physicians and provides the support needed between doctor’s visits. As a member of Care Improvement Plus, your clients can keep seeing their current doctors or choose any physician that accepts Medicare.

A Personal Support Team
A local Support Team will work closely with your clients, their physicians, and caregivers. The services include:

Access to staff nurses
Doctor appointment reminders
Medication monitoring and counseling
Education to promote self-care
Hospital discharge planning
In-home health monitoring equipment to help track symptoms (if qualified)
